New Student Enrollment
Enrolling in GCVS is easy – our public school is open to all Massachusetts residents who are in kindergarten through grade 12. New students will just need to fill out a few forms, including an enrollment form.
New students can fill out the enrollment form using the link to the right. After filling out the enrollment form, you will need to print and fill out the documents in the checklist, and return them by email or fax.

School Records Release
In order to best meet your student’s needs, we need to know the language (or languages) spoken in each students’ home. Please read and fill out the below form to help us best educate your child.
Free And Reduced Price School Meals
Please use the instructions below to help you fill out the application for free or reduced price school meals. You only need to submit one application per household. The application must be filled out completely to certify your children for free or reduced price school meals. Please follow these instructions in order! Each step of the instructions is the same as the steps on your application. If at any time you are not sure what to do next, please contact the GCVS office at 413-475-3879.
Proof of Residency
In addition to filling out and returning the residency affidavit below, you will need to supply proof of residency in the form of a utility bill with the address listed or a car insurance statement, Massachusetts car registration, and an excise tax bill.
If you are a tenant-at-will or family residing with someone else, you will need to provide a signed letter by the property owner that has been notarized by a notary public.
In the event of an emergency, we need to know emergency medical information, including any medical conditions and emergency contact information. Please fill out the below form and return it to us.
We will also need a copy of your student’s immunization records and a a completed student physical form.

- Proof of age – a birth certificate of other government-issued ID with verified date of birth
- Release of general and/or special education records
- Copy of current report card or transcript if available
- Home language survey
- Copy of Section 504 Plan and/or individualized education plan (IEP)
- Free and reduced lunch form
- Student physical form
- Immunization records
- Emergency medical information form
- Proof of residency
