Continuing students at GCVS have only a few forms to fill out in order to keep important information current.

Medical Information

In the event of an emergency, we need to know emergency medical information, including any medical conditions and emergency contact information. Please fill out the below form and return it to us.

We will also need a copy of your student’s immunization records and a a completed student physical form.

Free And Reduced Price School Meals

Please use the instructions below to help you fill out the application for free or reduced price school meals. You only need to submit one application per household. The application must be filled out completely to certify your children for free or reduced price school meals. Please follow these instructions in order! Each step of the instructions is the same as the steps on your application. If at any time you are not sure what to do next, please contact the GCVS office at 413-475-3879.

  • Free and reduced price school meals form
  • Proof of residency
    • Utility bill with address listed or a car insurance statement, Massachusetts car registration, and an excise tax bill.
    • If you are a tenant-at-will or family residing with someone else, you will need to provide a signed letter by the property owner that has been notarized by a notary public
